Return it to the LCBO and get your money back. If it's possible (see esprit's last post), then why not?

Has anyone actually done this? Just curious to learn whether anyone had any problems doing so.

I have had 12 Fred and 6 Doggie Claws and liked them all. I have some slight carbonation in the DC's and the Fred has been a crapshoot with the bad capping but so far the fluid is OK.

If the brewer is to be believed, the flatness was intentional.
It seems hard for me to believe the flatness was intentional. I could not get even a millimeter of head on this beer, even when i resorted to pouring it in a way that would normally give me a glass full of foam.

Well, the taste can be tamed (and isn't half bad) if you chill it to near-frozen. The problem with that is the beer will still warm up in your glass, so you have two choices. Drink it fast, or drink an increasingly crappy beer. At 10.5% ABV though, drinking it quickly will easily get you drunk.
