Actually, now that we have a whole section of the forums for private ordering, maybe we should make a sticky with all the information people will need for every order.

call me crazy, but i only see two: the order form in the word document (which seems to be a duplicate of the order form in the excel files) and the lab waiver (which is also in both excel files). doesn't seem to be anything to fill out/sign/return with the terms (also in both excel files). am i missing something?

- Email me at hairofthedogorder at gmail dot com with your name, phone number, and quantities so that I can compile a list to forward to Sylvia. (Thanks to those who have emailed already.) It's to help Sylvia know what orders to expect in the mail and to track down missing orders.

Just for the record, I added a case of Doggie Claws to my case of Fred. It's the fillng of the forms. The forms made me double my order.

I've just sent off my order, and should start drinking some of my stash to make room for the new stuff. Were that life was always so difficult...

BTW, on the chance that the LCBO once again cannot be arsed enough to send out an invoice, could you please be so kind as to post a PO number for this order once the beer comes in? I'm sure I'm not the only one here who'd appreciate it.

Thanks for everyone getting their info to me. FYI, here's a summary of what everyone told me they were ordering:

Adam: 10 cases
Fred: 12 cases
Rose: 4 cases
Ruth: 5 cases
Doggie Claws: 14

Grand Total: 45 cases! Nice!

Any update on this? Has Sylvia confirmed the order with you? It's one thing for people to e-mail you confirming their orders, it's another for her to have the order forms and deposits in hand.

I haven't heard any official response from Sylvia. I'll follow up with her to see what's going on.

Cool, thanks! Just wondering if I'm going to see my haul in time for Xmas. It's staggering to imagine but it's only 9 weeks away!

Sylvia called me twice last week - firston Tuesday to see if I had sent in my forms and then on Wednesday to say she just received my forms. I think mine was the last set of forms she needed to receive.

From what I remember Sylvia said she'd be sending something to us (via mail) giving details, etc. Hopefully that includes an idea of when the beer will get here.
